Root Cause Analysis Assessment

The RCA system is mainly divided into three major categories – RCA Management System, Investigation and Implementation. Further it’s divided into sub-categories: items and multiple questions are asked for each item. Compiled questions are being asked to organization’s employees who are associated and participated in the incident investigation. Over 300 compiled questions will be presented and asked to organization’s employees in different disciplines and roles such as Senior Management, Sponsors, Specialists, Leaders, Members and Trainees. The intention for this comprehensive interview process is to get multi-discipline views on organization’s RCA system and to help identify gaps between the current state and the target state.

Based on the interview responses, the different items of sub-categories and categories of RCA Management System, Investigation and Implementation will be given a grade from 1 to 10 and accordingly it’ll be rank as Poor, Fair, Good, Very Good and Excellent, and it is recognized The intent of these categories are to highlight where additional focus should be required, and where there may be potential issues as related to effective utilization of RCA Management System

RCA Management System

REC consultant will divide the RCA management system into sub-categories, items and each item will have a series of questions that will be asked to organization’s RCA team. Some of the sub categories of Procedure are General, RCA Department, Organization Management, Sponsor, Area Manager, Department Owner, RCA Specialist, Team Leader, Team Member and Issuer. Investigation Process

REC team of consultant will gather data by selecting/reviewing previous and current investigation reports, reviewing other documents and interviewing selected RCA sponsors, leaders and members by utilizing checklist and interview questionnaires.

Implementation:

REC expert consultant will evaluate previous RCA recommendation implementations activities and analyse the quality of each implementation by exploring solutions, key learning, tracking and statistical analysis. See below the pictorial representation of typical current state implementation.

Reporting

After completing of the RCA assessment, a detailed report with findings and solutions will be discussed with key personnel. It will be reviewed, documented and then presented to senior management.

Implementation

The road towards excellence begins with implementing the improvement initiatives outlined in the final report. These initiatives need to be carefully planned and coordinated to work within the constraints of your organizational resources. Implementation success is supported by change, to both work practices and behaviours. These changes need to be managed carefully if your investments are to deliver the desired results.

  • Developing a work plan:
    Implementing the recommendations through a predefined work plan.
  • Monitoring Report:
    A frequent monitoring report is generated to highlight accomplishment and areas needing improvement when the implementation is completed so that your organization can maintain complete control over future changes.
  • Statistical Analysis:
    Our consultants will do the study on the implemented recommendation and compare with gaps found during the process of the assessment.

Implementation will carry out by REC consultants with assigned resources and other consultants as needed.
